This article lists the steps required for an organisation to set up Checklists in Folio for use in Service Provider Reporting.
1) Create a Lite User for the Service Provider
The first step in setting up service provider reporting is creating a Lite User account that your Service Provider can use to submit their reports:

2) Create a Checklist for the Service Provider Report
The next step is to create a Checklist that will act as the Service Provider Report for the Lite User you have created:

3) Add questions to the Checklist
Now you need to add questions to the Checklist that you would like the Service Provider to answer for their report:

4) Set up Review Stages
Once a Service Provider has submitted a Checklist that was assigned to them, you can have multiple review stages set up where people from your organisation review the answers that the Service Provider has given before approving the report.

5) Assign Checklist to Service Provider
After you have finished setting up the Checklist and any review stages required, you can assign the Checklist to the Service Provider's Lite User. The Service Provider will get an email about the assignment and can log into Folio to complete it.

6) Complete Checklist Review Stages
Once the Service Provider has submitted the Checklist it will then move to it's first review stage, if any, and be assigned to the appropriate reviewer. The reviewer will get an email upon assignment and can then log into Folio to complete the review. This step will repeat until all review stages have been completed.
After all review stages are completed the Checklist is approved and the Service Provider Report is finished.
